To receive a refund, you must provide an application addressed to the individual entrepreneur (FOP) (the full name of the individual entrepreneur (FOP) indicated in the invoice must be specified,Appendix), requesting a refund according to paid invoice No. ________ dated ___.___.20___ in the amount of ____ UAH. (the application must be handwritten, signed and dated).
In the application, it is necessary to indicate the account number of the credit card from which the payment was made (funds are not returned to another card); the paid amount will be refunded to this card. It is also necessary to indicate the details of the bank through which the payment was made, namely: EDRPOU code, MFO and the bank account.
A scanned copy of the application (in electronic form) must be sent to the email address: booking@medvino.ua.
After verification of correctness, the original application must be sent to the postal address: Ukraine, Kyiv Region, Vyshhorod District, Medvyn village, 21 LISOVA ST.
After receiving the correctly completed refund application (original), the funds will be returned to the guest’s account from which the payment was made.
Refund rules for payment at a bank branch or through banking (in the app):
Application addressed to (depending on who received the payment): the individual entrepreneur (FOP) (the full name of the individual entrepreneur (FOP) indicated in the invoice must be specified, Appendix ), requesting a refund according to paid invoice No. ________ dated ___.___.20___ in the amount of ____ UAH. (the application must be handwritten, signed and dated). The application must indicate the card account number from which the payment was made (money is not returned to another card), and to which the paid amount will be refunded. It is also necessary to indicate the bank details through which the payment was made, namely: EDRPOU code, MFO and the bank account.
2. Copy of the 1st and 2nd pages of the passport of the guest who made the payment and the registration page. Copies must be certified with the signature of the holder, the date, and the written phrase “true to the original”
3. Copy of the guest’s identification number.
4. Original payment receipt:
– original payment receipt (when paying through a bank cashier)
– original payment order / bank statement with a wet seal, which can be obtained at the bank branch (when paying through “Internet banking”)
